Instructions

The following forms should be completed by a person who has a strong sense of the candidate. Please be sure to complete the forms as thoroughly and in as much detail as possible. This ensures the selection committee has the most information about the individual you are nominating.

The deadline for all nominations is April 1, 2024.

Nomination Forms

• Young Alumni Achievement Award: Alumni who are 40 years old or younger
• Outstanding Alumni Award
• Outstanding Faculty Award: Full professors
• Rising Star Faculty Award: Assistant and associate professors with 5-15 years of experience
