Dean's Certification

What is a Dean's Certification Form?

A Dean's Certification (or disciplinary clearance) is a verification of your student conduct record. Students requesting a Dean's Certification form must first fill out a Release of Academic Information Form, which permits the Cornell Office of Student Conduct and Community Standards and the Academic Integrity Board to release disciplinary information. 

What should be included in the email?

  • Full name
  • Student ID number
  • year and term of graduation from CALS
  • Current phone number
  • Unique handling information (if required)
  • PDF copy of the dean's certification form with your completed portion to cals-studentservices [at] cornell.edu (cals-studentservices[at]cornell[dot]edu)

How long does it take to process the form?

  • Generally processing Dean's Certifications can take 7-10 business days. Plan accordingly to submit the documents to meet deadlines.
  • Dean's Certifications are reviewed and completed once a week. Upon request, we will send you a confirmation email once we have sent out your certification.

Note: If the school the student is applying to did not send a specific form but is asking for a letter related to academic standing, please forward the email to cals-studentservices [at] cornell.edu (cals-studentservices[at]cornell[dot]edu)
