  • This form is required for any position change that incurs a change in funding (rate of pay, hours worked, etc.), change in title (re-class/promotion), or to establish a new position.   
  • Please note that if this request is to extend the term of a current position, and there is no change in title/pay/work location/duties/funding accounts, please consider using the term notification system rather than completing this form.
