Catastrophic Leave Donation Form

Cornell University provides catastrophic leave donations to assist eligible employees who have experienced a catastrophic, health-related event and have exhausted all accruals, except for up to five days of vacation, which they may elect to save. All non-academic and academic staff employees who accrue vacation after completing one year of university service may make donations as outlined in University Policy 6.9 - Catastrophic Leave Donation.

Confidentiality is an important aspect of the catastrophic leave donation program. Therefore, it is expected that all employees (e.g., donors, recipients, administrators, and supervisors), regardless of their decision to participate in a catastrophic leave donation, will do their part to maintain such confidentiality.

The maximum amount of vacation or health and personal leave that an employee may donate is ten days, in total, per fiscal year (July 1st – June 30th). The deadline for submitting donations is January 16th - September 15th to comply with University Policy 6.9 – Catastrophic Leave Donation regarding the acceptability of donations.
